Eligibility Criteria for Our Services

Our Care Home is committed to providing a safe, supportive, and compassionate residential environment for adults who require assistance with activities of daily living while maintaining the highest standards of individualized care. Admission is based on each prospective resident’s medical, functional, psychosocial, and financial assessment to ensure that the services provided meet the individual’s needs.

Medical

Have medication needs that can be safely managed

Have one or more chronic medical conditions requiring ongoing monitoring, medication management, or assistance with daily care.

Functional Eligibility

Require assistance with one or more Activities of Daily Living (ADLs)

Be ambulatory or have limited mobility that can be safely managed by staff. May use assistive devices.

Cognitive and Behavioral Eligibility

Have mild to moderate cognitive impairment.

Benefit from structured routines and supportive services.

Financial Eligibility

Have medication needs that can be safely managed

Pre-Admission Requirements

HattieCare Admission Policy

  • Have one or more chronic medical conditions requiring ongoing monitoring, medication management, or assistance with daily care.

  • Be medically stable and not require continuous hospitalization or acute medical intervention.

  • Have physician documentation supporting placement in a personal care or assisted living setting.

  • Be free from communicable diseases that pose a significant risk to other residents unless appropriately managed in accordance with public health guidelines.

  • Have medication needs that can be safely managed by the care home staff within the scope of Georgia regulations.

  • Diabetes Mellitus

  • Hypertension

  • Congestive Heart Failure (stable)

  • Chronic Kidney Disease (including outpatient dialysis patients)

  • Chronic Obstructive Pulmonary Disease (COPD)

  • Arthritis

  • Parkinson’s Disease (early to moderate stages)

  • Multiple Sclerosis (stable)

  • Stroke with residual deficits

  • Mild to Moderate Dementia or Alzheimer’s Disease

  • Anxiety and Depression that are stable with treatment

  • Other medically stable chronic illnesses requiring supportive care

The Care Home is generally unable to admit individuals who:

  • Are permanently bedridden.

  • Require total assistance with all transfers when such care exceeds facility capabilities.

  • Require continuous skilled nursing care beyond the scope permitted for a Georgia personal care home or assisted living community.

  • Are receiving hospice services requiring continuous bedside nursing beyond what can safely be coordinated within the home.

  • Have unstable or terminal medical conditions requiring frequent hospitalization or intensive medical intervention.

  • Require mechanical ventilation or other advanced life-support services not supported by the facility.

  • Pose a significant safety risk to themselves or others.

Admission decisions for residents receiving hospice services will be evaluated individually based on regulatory requirements, staffing capabilities, and the resident’s care needs.

Prior to admission, applicants must provide:

  • Physician’s medical evaluation.

  • Current medication list.

  • Medical history and diagnoses.

  • Tuberculosis screening or documentation as required by Georgia law.

  • Functional assessment.

  • Emergency contact information.

  • Insurance and financial information.

  • Copies of advance directives, if applicable.

  • Consent forms and admission agreements.

A comprehensive assessment may be completed by the facility prior to acceptance.

Accepted payment options may include:

  • Private Pay

  • Long-Term Care Insurance

  • Veterans Administration (VA) benefits, including Aid and Attendance when applicable

  • Georgia Medicaid programs, if accepted by the facility and the resident qualifies

  • Supplemental Security Income (SSI)

  • Social Security retirement or disability benefits

  • Georgia Community Care Services Program (CCSP), if contracted

  • Georgia SOURCE Program, if contracted

  • Georgia Independent Care Waiver Program (ICWP), if applicable

  • Georgia Elderly and Disabled Waiver Program (EDWP), if available

  • Other state or county assistance programs

  • Approved third-party payment arrangements

  • Guardian or conservator payment arrangements

  • Trust accounts or other approved funding sources

Financial eligibility and payment arrangements must be established before admission unless otherwise approved by administration.

Admission is contingent upon:

  • Successful completion of the pre-admission assessment.

  • Review of medical records.

  • Determination that the resident’s needs can be safely met.

  • Availability of appropriate accommodations.

  • Completion of all required admission documents.

  • Agreement to facility policies, resident rights, and financial obligations.

The Care Home reserves the right to deny admission if the applicant’s medical, behavioral, or functional needs exceed the services the facility is licensed or equipped to provide.
